Application Submission Portal

Thank you for your interest in our School's undergraduate programs. Prospective Public Health BA and Public Health BS majors must submit their applications during the Spring application period. You may submit only one application to one program (BA or BS) per application period.

The application asks you for the following information:

1. Name, UID, preferred email address, and information about planned absences
2. List of preparation for the major courses you have completed or are currently taking
3. Short response questions (each response: 1500-2500 characters; 300-500 words)
4. Resume (PDF only)

Please visit the FSPH Office of Undergraduate Student Services Google site for more information about the major admissions process, including the short response questions and FAQs.

The application should be completed in one session, so please be ready to provide your responses to questions and have your resume ready to upload.

The admissions committee will review the materials you submit, as well as your UCLA transcript.
