Application Submission Portal

Thank you for your interest in our School's undergraduate programs. Prospective Public Health BA and Public Health BS majors must submit their applications during the Winter application period. Prospective Public Health minors can apply during Fall and Winter . You may submit only one application to one program per application period.

The application asks you for the following information:

1. Name, UID, preferred email address, and information about planned absences
2. List of Fielding School of Public Health courses completed or currently taking
3. Short response questions (each response: 1500-2500 characters; 300-500 words)
4. Resume (PDF only)

Please visit the FSPH Office of Undergraduate Student Services Google site for more information, including the short response questions for each program's application.

The application should be completed in one session, so please be ready to provide your responses to questions and have your resume ready to upload.

The admissions committee will review the materials you submit, as well as your UCLA transcript.
